Job summary

Optum Home & Community Care, part of UnitedHealth Group, seeks an Advanced Practice Clinician for our Senior Community Care program. You will assess, plan and coordinate care for high-risk members in nursing homes, senior housing and home care settings.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active, unrestricted licensure in practicing state.
• Active and unrestricted driver's license with reliable transportation.
• Current, active DEA licensure/prescriptive authority or ability to obtain post-hire.
• Ability to gain a collaborative practice agreement where applicable.
• Ability to navigate varied environments and perform job duties.

Responsibilities

• Perform comprehensive assessments including medical history review, vitals, exams and screenings.
• Support members during sub-acute rehabilitation episodes.
• Ensure accurate and complete documentation (ICD-10).
• Collaborate with interdisciplinary care team.
• Lead advanced illness and end-of-life care planning conversations.
• Counsel patients and caregivers with education and resources.
