United Family Medical Center seeks a compassionate Nurse Practitioner to provide care to residents in skilled nursing facilities in the Amesbury and Haverhill area. The role offers the chance to build meaningful patient relationships and collaborate with facility teams to deliver high-quality care.

Responsibilities include patient assessments, treatment planning, chronic condition management, and medication management, with teamwork among physicians, nurses, and rehabilitation staff.

Qualifications

• Active NP license in Massachusetts.
• Board certification in FNP, ANP, or relevant specialty.
• Valid DEA license and prescriptive authority in Massachusetts.
• Experience in primary care or family medicine preferred.
• Strong clinical assessment, critical thinking, and diagnostic skills.
• Excellent communication and compassionate patient-first approach.

Responsabilités

• Conduct patient assessments, develop treatment plans, and manage acute and chronic conditions.
• Provide ongoing care, follow-up visits, and medication management.
• Collaborate with physicians, nurses, rehabilitation staff, and other care team members.
• Support transitions of care and communicate with patients and families.
• Maintain accurate and timely clinical documentation.
