Job summary

Qualifications
- Active NP license in Massachusetts.
- Board certification (FNP/ANP or related).
- DEA license and prescriptive authority in Massachusetts.
- Experience in primary care or family medicine preferred.
- Strong clinical assessment and communication skills.
Responsibilities
- Conduct patient assessments, develop treatment plans, and manage acute and chronic conditions.
- Provide ongoing care, follow-up visits, and medication management.
- Collaborate with physicians, nurses, rehabilitation staff, and other care team members.
- Support transitions of care and communicate with patients and families.
- Maintain accurate and timely clinical documentation.
