Job summary

Reliant Medical Group, part of the Optum family, seeks a 0.6 FTE Advanced Practice Clinician (NP/PA) to join our Skilled Nursing Facility team in Worcester, MA. The role emphasizes value-based care for older adults and collaboration with a physician colleague to meet goals of care.

You will lead a multidisciplinary team, monitor both acute and chronic conditions, and provide longitudinal care to long-term residents within a supportive, clinician-led organization.

Qualifications

• National Board Certification is required.
• Licensed by the Commonwealth of Massachusetts Board of Registration of Nursing.
• Graduate of an Accredited Nurse Practitioner Program.
• Maintains BLS certification and professional certification as appropriate.

Responsibilities

• Provide value-based care to older adults on skilled rehab and long-term care units.
• Lead multidisciplinary teams including nursing, dietary, social work, PT, OT, SLP and case management.
• Collaborate with a physician colleague to align goals of care (recovery, palliative or comfort care).
• Monitor acute and chronic conditions common in nursing homes (e.g., dementia, Parkinson's).
• Perform longitudinal follow-up and care for long-term residents per state regulations.
