Job summary

Clover Health is seeking a compassionate Nurse Practitioner to join our Home Care Team in Georgia. This hybrid role combines in-home visits with telehealth care for Medicare Advantage members across multiple markets in Georgia.

You'll perform comprehensive assessments, manage chronic diseases, and develop individualized care plans while addressing social determinants of health. Collaboration with an interdisciplinary team and use of Clover Assistant are essential.

Qualifications

• Active NP license in Georgia.
• DEA registration required.
• National board certification as NP.
• Experience with adult/geriatric populations in primary, chronic, home-based, or value-based care.
• Strong clinical assessment and chronic disease management skills.
• Home-based or population health experience preferred.
• Comfort with telehealth technology.
• Excellent communication and collaboration skills.

We are seeking a compassionate and driven Nurse Practitioner to join our growing Clover Home Care Team in Georgia.

This hybrid role supports Medicare Advantage members throughout Georgia, combining in-home visits with telehealth care.

You will provide care across the Savannah, Athens, Rome, Macon, Augusta, Thomasville, Chatham, Columbus, Brunswick, and Atlanta markets.

This is an opportunity to build meaningful relationships with patients while improving outcomes for medically complex and vulnerable senior populations.

What You'll Do

• Conduct in-home and telehealth visits for Medicare Advantage members.
• Perform comprehensive assessments, medication reviews, and chronic disease management.
• Develop and implement individualized care plans that address both clinical and social determinants of health.
• Identify barriers to care, including fall risks, food insecurity, caregiver challenges, and gaps in treatment adherence.
• Collaborate with physicians, care coordinators, social workers, and other interdisciplinary team members to deliver holistic, member-centered care.
• Leverage Clover Assistant and EMR technology to support proactive care management and close gaps in care.
• Monitor health status and intervene early to help prevent avoidable emergency department visits and hospitalizations.
• Educate members and caregivers on disease management, preventive care, and treatment plans.

Qualifications

• Active Nurse Practitioner (NP) license in Georgia or ability to obtain licensure.
• You have an active DEA registration.
• National board certification as a Nurse Practitioner.
• Experience caring for adult or geriatric populations in primary care, complex care, chronic care management, transitional care, home-based care, or value-based care settings.
• Strong clinical assessment and chronic disease management skills.
• Passion for serving vulnerable senior populations.
• Home care, home health, palliative care, geriatrics, or population health experience preferred.
• Comfortable utilizing technology and telehealth platforms to enhance patient care.
• Excellent communication and collaboration skills.

Preferred Qualifications

• Experience in home-based care, house calls, home health, or population health management.
• Experience managing patients with complex and chronic medical conditions.
• Comfort with telehealth technology and hybrid care delivery models.
