This hybrid role supports Medicare Advantage members throughout Bergen, Burlington, Middlesex, Hudson, and Ocean Counties, combining in-home visits with telehealth care. This is an opportunity to build meaningful relationships with patients while improving outcomes for medically complex and vulnerable senior populations.

Final pay is based on several factors including but not limited to internal equity, market data, and the applicant’s education, work experience, certifications, etc.
A reasonable estimate of the base salary range for this role is:
$155,000—$165,000 USD
