Job summary

Clover Health is seeking a compassionate Nurse Practitioner for a hybrid Home Care role in New Jersey. You will perform in-home visits and telehealth for Medicare Advantage members across multiple counties, building personalized care plans and coordinating with a multidisciplinary team.

You will use Clover Assistant and EMR technology to monitor health, close care gaps, and educate patients and caregivers on disease management and preventive care. Travel throughout the service area is required.

Qualifications

• Active NP license in New Jersey or ability to obtain licensure.
• National board certification as a Nurse Practitioner.
• Active DEA and New Jersey CDS registrations, or ability to obtain.
• Experience caring for adult or geriatric populations in primary, chronic, home-based, or value-based care settings.
• Strong clinical assessment and chronic disease management skills.

Responsibilities

• Conduct in-home and telehealth visits for Medicare Advantage members.
• Perform comprehensive assessments, medication reviews, and chronic disease management.
• Develop and implement individualized care plans addressing clinical and social determinants of health.
• Identify barriers to care including fall risks, food insecurity, caregiver challenges, and adherence gaps.
• Collaborate with physicians, care coordinators, social workers, and the interdisciplinary team to deliver holistic care.
• Leverage Clover Assistant and EMR to support proactive care management and close gaps in care.
• Monitor health status and intervene early to prevent avoidable ER visits and hospitalizations.
• Educate members and caregivers on disease management, preventive care, and treatment plans.

We are seeking a compassionate and driven Nurse Practitioner to join our growing Clover Home Care Team in New Jersey.

This hybrid role supports Medicare Advantage members throughout Bergen, Burlington, Middlesex, Hudson, and Ocean Counties, combining in-home visits with telehealth care. This is an opportunity to build meaningful relationships with patients while improving outcomes for medically complex and vulnerable senior populations.

What You'll Do

Qualifications

• Active Nurse Practitioner (NP) license in New Jersey or ability to obtain licensure.


• National board certification as a Nurse Practitioner.


• Active DEA and New Jersey CDS registration, or ability to obtain.


• Experience caring for adult or geriatric populations in primary care, complex care, chronic care management, transitional care, home-based care, or value-based care settings.


• Strong clinical assessment and chronic disease management skills.


• Passion for serving vulnerable senior populations.


• Comfortable utilizing technology and telehealth platforms to enhance patient care.


• Excellent communication and collaboration skills.

Preferred Qualifications

• Home care, home health, home-based primary care, palliative care, geriatrics, or population health experience.


• Experience caring for Medicare Advantage members.


• Prior experience in value-based care models.


• Experience managing medically complex and high-risk patient populations.


• Bilingual skills are a plus.
