About the Position:

During visits that can last up to one hour, Matrix providers review and observe a member’s current health, medical history, medication adherence, social environment, and other risks. This provides unmatched insight into a member’s overall health and well-being that can be difficult to capture during routine office visits. The Matrix Comprehensive Health Assessment helps improve quality of care and can close multiple care gaps with a single visit.

Responsibilities:

What to Expect:

• Conduct adult/geriatric assessments, medical histories, diagnoses, treatments, health education, referrals, follow-ups, and documentation according to Matrix guidelines
• Collaborate with physicians, case managers, social workers, family members, and other medical personnel
• Coordinate with Primary Care Physician (PCP) on patient education and follow-up
• Provide services in various venues including home visits and skilled facility visits

Position Details:

Type: Full Time Permanent

Location: Home, SNF, and other community environments. Travel required throughout the state and neighboring states based on business needs.

Hours: Monday – Friday, 8:00 AM – 5:30 PM (hours may vary; full-time work expected)

Salary: $129,600 - $144,500 per year, based on experience

Qualifications:

• Master’s Degree or equivalent experience with NP licensure
• Current RN and NP licensure in the state of practice, including prescription authority or ability to obtain it
• Board certification by AANP, ANCC, or AACCN in a Matrix-approved specialty
• Current BLS, ACLS, or CPR certification
• At least 3 years of experience as a Nurse Practitioner
• Strong computer skills and familiarity with medical record software
• Excellent communication skills (verbal and written)
• Adaptability to frequent changes

Travel Requirements:

• Travel required to meet patients across the assigned state and neighboring states
• Willingness to travel up to 80-90% of the time, including weekends and overnight stays
• Valid driver’s license, ability to drive, and proof of adequate auto insurance coverage
