About the Position:

During a visit that can last up to one hour, Matrix providers review and observe a member’s current health, medical history, medication adherence, social environment, and other risks. This provides unmatched insight into a member’s overall health and well-being that can be difficult to capture during routine office visits. The Matrix Comprehensive Health Assessment helps to improve quality of care and allows us to potentially close multiple care gaps with a single visit.

Responsibilities:

What to Expect:

• Conduct adult/geriatric assessments, medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols
• Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and other medical personnel as appropriate
• Collaborate with Primary Care Physician (PCP) on patient education and provide follow-up
• Provide services in various venues, including home visits and skilled facility visits

Position Details:

Nurse Practitioner - Full Time, Regional Traveler - Home Risk Assessments

Type: Full Time Permanent

Location: Home, SNF, and other community environments across the state and neighboring states (travel required)

Hours: Monday – Friday, 8:00 AM – 5:30 PM (hours may vary, full-time work expected)

Salary: $129,600.00 - $144,500.00 annually, based on experience

Qualifications:

Required:

• Master’s Degree or equivalent experience with NP licensure
• Current RN and NP licensure in the practicing state, including prescriptive authority or ability to obtain it
• Board certification by AANP, ANCC, or AACCN in an approved specialty
• Current BLS, ACLS, or CPR certification
• Minimum of 3 years experience as a Nurse Practitioner
• Strong computer skills and familiarity with medical record software
• Excellent communication skills with patients and colleagues
• Flexibility to adapt to frequent changes

Travel Requirements:

• Willingness to travel across the state and neighboring states to meet patients at their locations
• Availability to travel 80-90% of the time, including overnight and weekends
• Valid driver’s license, ability to drive, and proof of adequate auto insurance
