About the Position:

During visits that can last up to one hour, Matrix providers review and observe a member’s current health, medical history, medication adherence, social environment, and other risks. This provides unmatched insight into a member’s overall health and well-being that can be difficult to capture during routine office visits. The Matrix Comprehensive Health Assessment helps improve quality of care and allows us to potentially close multiple care gaps with a single visit.

Responsibilities:

What to Expect:

• Conduct adult/geriatric assessments, medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up, and documentation according to Matrix guidelines and protocols.
• Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
• Collaborate with Primary Care Physician (PCP) on patient education and provide follow-up.
• Provide services in various venues, including home visits and skilled facility visits.

Position Details:

Type: Full Time Permanent

Location: Home, SNF, and other community environments across the state and neighboring states (travel required).

Hours: Monday – Friday, 8:00 AM – 5:30 PM (hours may vary; full-time work expected).

Salary: $129,600 - $144,500 annually, depending on experience.

Qualifications:

Required:

• Master’s Degree or equivalent experience with NP licensure.
• Current RN and NP licensure in the state of practice, including prescription authority or ability to obtain it.
• Board certification by AANP, ANCC, or AACCN in an approved specialty.
• Current BLS, ACLS, or CPR certification.
• At least 3 years of experience as a Nurse Practitioner.
• Strong computer skills and familiarity with medical record software.
• Excellent communication skills.
• Flexibility to adapt to frequent changes.

Travel Requirements:

• Travel required to meet patients across the state and neighboring states as needed.
• Willing to travel up to 80-90% of the time, including overnight and weekend travel.
• Valid driver’s license, ability to drive, and proof of adequate automobile insurance.
