Job summary

Oak Medical SC in Madison, WI seeks a compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to provide acute and chronic care management for adult and geriatric patients across Madison Area, including Beloit, Watertown, Lake Mills, Mt. Horeb, Lodi, DeForest.

Roles include covering PTO, collaborating with physicians and care teams to implement treatment plans that improve outcomes and quality of life. Flexible, weekday schedule with RVU bonus opportunities and supportive leadership.

Qualifications

• Current Nurse Practitioner or Physician Assistant license.
• Active DEA registration.
• BLS certification.
• Strong clinical assessment and diagnostic skills.
• Ability to work independently while collaborating effectively with interdisciplinary teams.
• Commitment to compassionate, patient-centered care.

Responsibilities

• Provide primary care services for adult and geriatric patients with acute and chronic medical conditions.
• Evaluate patients, establish diagnoses, prescribe medications, and perform diagnostic and therapeutic procedures within scope of practice.
• Develop individualized treatment plans in collaboration with physicians and interdisciplinary care teams.
• Coordinate care with specialty providers, including wound care, psychiatry, and other consulting services.
• Communicate changes in patient condition and treatment plans with patients, families, facility staff, and healthcare team members.
• Maintain timely, accurate, and compliant documentation.
• Build strong relationships with patients, families, and facility staff to support exceptional care.
• Perform other duties as assigned.

What You'll Do

• Provide primary care services for adult and geriatric patients with acute and chronic medical conditions.

1 Required
