Job summary

Oak Medical SC seeks a compassionate Nurse Practitioner or Physician Assistant to provide acute and chronic care management for adult and geriatric patients across the Madison Area, including Beloit, Watertown, Lake Mills, Mt. Horeb, Lodi, and DeForest.

We offer 1 Part-Time and 1 Full-time Float positions with a consistent Monday–Friday schedule from 7:00 AM to 5:00 PM, no nights, weekends, or holidays, and strong collaboration with physicians and care teams to implement comprehensive treatment

Qualifications

• Current Nurse Practitioner or Physician Assistant license.
• Active DEA registration.
• BLS certification.
• Strong clinical assessment and diagnostic skills.
• Ability to work independently while collaborating effectively with interdisciplinary teams.
• Commitment to compassionate, patient-centered care.

Responsibilities

• Provide primary care services for adult and geriatric patients with acute and chronic conditions.
• Evaluate patients, establish diagnoses, prescribe medications, and perform diagnostic and therapeutic procedures within scope.
• Develop individualized treatment plans in collaboration with physicians and interdisciplinary teams.
• Coordinate care with specialty providers, including wound care, psychiatry, and other services.
• Communicate changes in patient condition and treatment plans with patients, families, facility staff, and healthcare team members.
• Maintain timely, accurate, and compliant documentation.

While covering colleagues on PTO, you'll collaborate with physicians, specialists, and facility care teams to develop and implement comprehensive treatment plans that improve patient outcomes and quality of life.

What You'll Do

• Provide primary care services for adult and geriatric patients with acute and chronic medical conditions.

• Build strong relationships with patients, families, and facility staff to support exceptional care.

Qualifications

Required

• Current Nurse Practitioner or Physician Assistant license.
• Active DEA registration.
• BLS certification.
• Strong clinical assessment and diagnostic skills.
• Ability to work independently while collaborating effectively with interdisciplinary teams.
• Commitment to compassionate, patient-centered care.

Preferred

• Leadership experience or demonstrated leadership skills.
• Strong organizational and time-management abilities.
• Positive, solution-oriented attitude.
• Experience working with adult and geriatric populations.
