Job summary

Oak Medical SC is seeking a Part-Time PMHNP to provide psychiatric evaluations and medication management across adult and geriatric populations in Oshkosh, Fox Valley, Green Bay, and Manitowoc. Qualifications

• Active PMHNP certification.
• Active DEA registration.
• Current BLS certification.
• Experience performing psychiatric evaluations and medication management.
• Strong diagnostic and treatment planning skills in behavioral health.
• Ability to work independently across multiple care settings.
• Strong collaboration and communication skills.
• Commitment to high-quality, patient-centered psychiatric care.

Responsibilities

• Deliver psychiatric evaluations and ongoing medication management for adult and geriatric patients.
• Diagnose and treat mental health disorders and dementia-related conditions.
• Perform comprehensive psychiatric assessments and develop individualized treatment plans.
• Order and interpret appropriate diagnostic testing.
• Prescribe medications and adjust treatment plans as clinically indicated.
• Manage a patient panel of approximately 16 patients per day across multiple sites.
• Collaborate closely with physicians, care teams, and facility staff.
• Participate in clinical team meetings and care coordination discussions.
• Ensure accurate, timely documentation and billing compliance.
• Apply strong clinical judgment in both routine and complex situations.
