Job description

Role Overview:

As a Nurse Practitioner in a senior-focused primary care setting, you will deliver comprehensive health promotion, diagnosis, and treatment for acute and chronic conditions. You will prescribe medications, examine patients, coordinate services, and support a team-based care model led by physicians.

Key Responsibilities

• Evaluate and treat patients according to care standards
• Monitor care quality through data and chart reviews
• Coordinate patient services, including referrals and home care
• Lead and participate in daily team huddles
• Complete medical documentation accurately and timely
• Ensure compliance with policies and participate in service growth opportunities
• Contribute to quality improvement and participate in on-call programs

Qualifications & Skills

• 2-5 years of relevant experience
• Board-certified nurse practitioner with active, unrestricted license
• Strong communication and interpersonal skills
• Ability to adapt to a value-based care environment
• Proficiency with EMR and basic computer skills
• Certification to prescribe medication and TB screening eligibility

Preferred Qualifications

• Active DEA, Medicare, and Medicaid provider numbers
• Experience managing Medicare Advantage panels and understanding Medicare guidelines
• Knowledge of HEDIS quality indicators
