The role involves evaluating patients, coordinating home health services, and participating in team-based care with minimal on-call requirements.

• Board-certified Nurse Practitioner or Physician Assistant with active license.
• 2-5 years of relevant experience preferred.
• Excellent communication and interpersonal skills.
• Ability to adapt to value-based care models.
• Basic computer proficiency and certification for prescribing medications.
• Willingness to participate in TB screening and maintain licensing standards.
