Job summary

Senior Doc is seeking a Float Nurse Practitioner (NP) or Physician Assistant (PA) to deliver mobile primary care to seniors in homes, boarding cares, and senior living communities. This role supports PTO, turnover, and new building launches while maintaining continuity of high-quality, relationship-based care.

The ideal candidate holds an active NP/PA license, has 2+ years in geriatrics or primary care, and can travel regionally.

Qualifications

• Active NP or PA license required.
• 2+ years experience in geriatrics, primary care, home health, SNF, or senior living.
• Comfortable practicing independently in mobile/community settings.
• Strong clinical judgment and panel management skills.
• Adaptable, organized, growth-oriented.
• Valid driver’s license and ability to travel within region.

Responsibilities

• Provide mobile primary care to seniors in homes, boarding cares, ALF, SNF, and hospice settings
• Step into established panels to ensure seamless patient continuity
• Support new market expansion and building launches
• Manage chronic conditions (HTN, CHF, COPD, diabetes, dementia, polypharmacy)
• Deliver dementia-informed, relationship-based care
• Collaborate with interdisciplinary partners (psychiatry, palliative care, neurology)
• Maintain strong documentation, communication, and quality standards

We are hiring a Float Advanced Practice Provider (NP/PA) to support a large regional footprint — providing coverage for PTO, turnover, and new building launches while maintaining continuity of high-quality, relationship-based care.
