Job summary

UnitedHealth Group seeks an Advanced Practice Clinician (NP or PA) to conduct comprehensive in-home health assessments for Medicare Advantage and other members.

The role is field-based, non-prescribing, focusing on education, gap closure, and collaboration with primary care providers to improve outcomes. Requires NP/PA certification and state licensure, driver’s license, and ability to work across varied home environments.

Qualifications

• Active NP certification (ANCC/AANP) for Family/Adult/Geriatric or Adult-Gerontology Primary Care.
• Current NCCPA certification for PAs and state licensure.
• Active, unrestricted state licensure where practicing.
• Active and unrestricted driver's license with access to reliable transportation for home visits.

Responsibilities

• Conduct in-home assessments including medical history, med reconciliation, vitals and exam.
• Identify and document diagnoses for care management and treatment planning.
• Communicate findings to PCPs to address gaps in care and coordinate care.
• Recognize urgent situations and intervene appropriately.
• Educate members on disease processes, medications and adherence.
• Address social determinants of health and provide referrals as needed.
