Qualifications

• 1+ years of clinical experience in family, geriatric, or home health preferred.
• Proficiency with electronic medical records and technology.
• Strong communication skills with geriatric or Medicare populations.

Responsibilities

• Conduct in-home health assessments for Medicare Advantage and other plan members.
• Review past medical history, medications, vital signs, and perform physical exams.
• Educate members on disease processes, medications, and adherence; coordinate with PCPs.
