Chase Brexton Health Services in Baltimore, MD seeks a Nurse Practitioner Fellow as part of the SNAPT Fellowship Program. This post-graduate trainee will provide mentored clinical care to patients and participate in an interdisciplinary team, with a transition to traditional practice after 12 months and potential for a second year.

Qualifications

  • Recent graduate (within 2 years) of an accredited Nurse Practitioner program.
  • Primary Care Adult or Family certification, or eligibility for certification prior to start of Fellowship.
  • Maryland licensure and clinical certification, eligibility for DEA and Maryland CDS.
  • Interest in and/or experience in community health settings.
  • Interest in and/or experience in gender affirming care, HIV medicine, sexual health.
  • Experience working in a diverse environment.
  • Excellent communication and team skills required.

JOB SUMMARY:

The Nurse Practitioner Fellow is a post-graduate clinical trainee participating in the SNAPT Fellowship Program who provides mentored clinical care to patients through an interdisciplinary and collaborative approach that considers the psychosocial aspects of health and disease. The Fellow will transition after 12 months to traditional practice and is committed to a second year of employment in a Nurse Practitioner role with opportunities for ongoing employment.

MAJOR DUTIES AND RESPONSIBILITIES:
Analytical and Critical Thinking
  • Maintains the standard of clinical care, using diagnostic testing appropriately and providing appropriate treatment
  • Considers the psychosocial aspects of both health and disease
  • Uses diagnostic testing appropriately yet cost-effectively, and providing appropriate treatment
  • Self-assessment and reflection on individual learning objectives, competencies, and goals
  • Effectively engages patients in their care
Checking, Examining, and Recording
  • Maintains complete and comprehensive documentation
  • Participates in evaluation and feedback processes with preceptors and mentors
Professionalism
  • Seeks physician consultation when appropriate
  • Demonstrates professionalism and integrity in all aspects of patient care
  • Shows meaningful progress toward professional role development
Compliance, Policy, and Procedure
  • Shows meaningful progress toward quality improvement goals
SKILLS AND ABILITIES:
  • Ability to work with a multidisciplinary clinical team
  • Computer skills including Microsoft office suite, electronic medical records and practice management system
EDUCATION AND/OR EXPERIENCE:

WORKING CONDITIONS/PHYSICAL DEMANDS:
  • Work is typically performed in a clinical and office environment. Travel among Chase Brexton Health Services clinics is required. Exposure to communicable diseases or hazardous material is possible, with little likelihood of harm if established precautions are followed.
  • Ability to perform functions normally completed during clinical care (i.e., sitting, standing, fine motor coordination), use of the keyboard for typing or editing EMR notes, clinically evaluating and communicating plans to patients.
