Job summary

LifeBridge Health in Baltimore seeks a full-time Family Nurse Practitioner to join Grace Medical Center Ambulatory Services, supporting population health initiatives in Baltimore City and County.

The NP will deliver primary care in Grace Medical Center's Family Medicine practice and the LBH Mobile Van, with emphasis on preventive care and chronic disease management, school visits, and community partnerships.

Qualifications

• Master's degree in Nursing or related advanced practice nursing degree.
• Active Advanced Practice Nurse license (APN/APRN/NP).
• Maryland DEA & CDS license or ability to obtain.
• CPR certification and continuing education requirements.

Responsibilities

• Provide primary care in Grace Medical Center's Family Medicine practice and via the LBH Mobile Van.
• Visit schools and community partners to deliver preventive and primary care services.
• Support practice growth and continuity of care within the health system.
• Coordinate with EMR systems remotely when needed.

LifeBridge Health is seeking a full-time Family Nurse Practitioner to join our Grace Medical Center Ambulatory Services team supporting population health initiatives in Baltimore City and Baltimore County.

The qualified Nurse Practitioner will collaborate on the development of growing population health services and community partnerships.

The Opportunity

• This full-time position will provide primary care services both in the community through the LBH Mobile Van and within Grace Medical Center's Family Medicine practice.
• The selected Nurse Practitioner will visit schools and other community partners throughout Baltimore City and Baltimore County to deliver preventative and primary care services, including well-child and adult wellness visits, sports physicals, school vaccinations, chronic disease management, and primary and sick care.
• The NP will also see patients in the Family Medicine practice to support practice growth, expand access, and established continuity of care.
• Remote access to EMR system, laptop computer, and printer when on the Mobile Van.
• No on-call responsibilities and no regularly scheduled evening or weekend hours; however, occasional weekend availability may be required to support community events, outreach activities, or special vaccination initiatives.
• Identify patients needing ongoing primary care services and connect them with Grace Medical Center's Ambulatory Services and Family Medicine practice to support continued care and continuity within the health system.

Requirements

• Master's degree and Advanced Practice Nurse, Nurse Practitioner license (APN, APRN, or NP)
• One - two years' NP experience required
• Demonstrated computer proficiency. Excellent documentation and organizational skills.
• Maryland DEA & CDS license (or obtainable) and current CPR license.
• Excellent interpersonal and human relations skills
• Demonstrated proficiency in direct patient care
• Ability to work collaboratively with all members of the health care team, patients, and families
• Knowledge of regulatory and accrediting agency standards and requirements (i.e., vaccines for children)
• Knowledge of sound nursing and medical practices
