Job summary

Trio Health Partners is seeking a Weekend In-House Nurse Practitioner to support the primary internal medicine provider in a Skilled Nursing Facility (SNF). You will build strong relationships with residents, families, facility staff, and other providers while leading clinical care as part of the team.

Qualifications

• Active Illinois Nurse Practitioner license (unencumbered).
• Master's Degree in Nursing from an accredited program.
• Strong clinical background in primary care and preventive medicine.
• Geriatric or adult medicine experience preferred, with focus on skilled nursing/long-term care.

Responsibilities

• Ensuring continuity of care by communicating with the primary In-House NP.
• Conducting new admission evaluations and monthly stability visits for long-term residents.
• Performing comprehensive physical examinations and evaluating changes in patient conditions.
• Prescribing medications and treatments as needed.
• Developing patient care policies and facilitating referrals to specialists.
• Coordinating advanced care planning and discharge visits.
• Ensuring timely communication and documentation of patient assessments and follow-ups.
• Supporting the physician-patient relationship and addressing gaps in care.

As a Weekend In-House Nurse Practitioner, you will be supporting the primary provider of internal medicine services within a Skilled Nursing Facility (SNF). Job Requirements

• Active Illinois Nurse Practitioner License (unencumbered).
• Master's Degree in Nursing from an accredited program.
• Strong clinical background in primary care and preventive medicine.
• Geriatric or adult medicine experience preferred, with a focus on skilled nursing, long-term care, hospice, palliative care, or similar settings.
• Ability to engage and communicate effectively with elderly and chronically ill patients and their families.
• Ability to maximize onsite time to develop robust caseloads
• Knowledge of Medicare, Medicaid, and Health Plan benefit structures is a plus.
