Creative Solutions Services, LLC seeks an experienced Nurse Practitioner (NP) for a contract position at our Corporate Health Center in Hammond, IN. The NP will perform physical examinations, order tests, and collaborate with Physicians to develop comprehensive patient care plans.

Requirements include MSN, state NP license, national NP certification, and BLS; Epic proficiency is preferred. This is a fast-paced contract with an ASAP start and an end date of 12/31/2026, with potential extension.

Qualifications

• MSN degree required for the role.
• Nurse Practitioner license in the applicable state is mandatory.
• National certification as Family or Adult NP required.

Responsibilities

• Performs physical examinations and preventive health measures within guidelines.
• Orders, interprets, and evaluates diagnostic tests to assess needs.
• Records findings and develops care plans and prognosis.
• Provides written home-going instructions to patients.
• Collaborates with physicians and health professionals on care plans.
• Prescribes medications or therapies as appropriate.
