Qualifications

• Active Nurse Practitioner license to practice in the state of Indiana.
• BLS certification required at hire date.
• State CDS registration and DEA registration current.

• Conducts physical exams and preventive health assessments.
• Performs procedures such as injections, wound care, and suturing.
• Develops and documents patient care plans and progress notes.
• Educates patients on treatment plans and health maintenance.
• Orders and interprets diagnostic tests; collaborates with physicians.
