Responsibilities

• Recognizes conditions requiring referrals to specialists.
• Manages chronic conditions such as diabetes and hypertension.
• Prescribes medications and treatments as appropriate.
• Advises on disease prevention and healthy behaviors.
• Refers patients to community resources for support.
• Consults with collaborating physicians as needed.
• Documents care plans thoroughly and timely.
• Completes forms for patient care and billing as required.
• Performs other duties as assigned.
