Union Community Care is seeking a Physician to provide patient-centered medical care as part of a collaborative care team in Pennsylvania. The physician will conduct examinations, develop treatment plans, and coordinate with specialists as needed, while ensuring proper documentation and adherence to guidelines.

The role requires PA licensure, board certification or eligibility in the specialty, and strong English communication.

• MD/DO licensed in Pennsylvania
• Board Certification or eligibility in their specialty
• English proficiency required

Responsabilités

• Provides patient-centered, culturally sensitive primary medical care to Union patients.
• Examination, treatment and documentation of the care plan per professional standards.
• Refer patients to medical specialists or other sources of service when necessary.
• Prescribes and/or dispenses medications according to prescriptive authority and state regulations.
• Performs examinations and formulates treatment plans with appropriate tests and referrals.
• Educates patients about their health and wellness and supports care team education.
