Job summary

Neighborhood Outreach Access to Health (NOAH) in Phoenix seeks a Nurse Practitioner to provide and manage direct medical evaluation, diagnosis, and treatment in accordance with evidence-based guidelines.

You will prescribe medications, review and update medical records, and coordinate patient care with specialists while engaging patients and families in decision-making and wellness education.

Qualifications

• Master’s degree in nursing-related field from NLN/CCNE.
• National Certification as Nurse Practitioner.
• Arizona Board of Nursing scope of practice for NPs.
• DEA certification.
• BLS certification.

Responsibilities

• Provide direct patient care including examinations, evaluations, diagnoses, and treatment per evidence-based guidelines.
• Prescribe medications and treatment regimens as appropriate.
• Review and update medical records per standards and regulations.
• Refer patients to specialists as needed and coordinate care.
• Counsel patients and families on wellness, prevention, and health maintenance.
• Direct and coordinate nursing and support staff in patient care activities.
• Follow PPE and safety standards and participate in trainings and meetings.
