CHRISTUS St. Vincent's Center for Healthy Aging in Santa Fe, New Mexico seeks a compassionate Nurse Practitioner to deliver outpatient geriatric care that supports independence, function, and quality of life.

You will conduct annual wellness visits, manage acute and chronic conditions, and collaborate with an interdisciplinary team. The role offers flexible scheduling (nine-to-five or four 10-hour days), competitive pay, and benefits from day one, plus state loan forgiveness opportunities.

Formación

• Minimum two years of Nurse Practitioner experience.
• Active New Mexico NP license, or ability to obtain.

Responsabilidades

• Conduct annual wellness visits and preventive health screenings.
• Evaluate, diagnose and treat acute and chronic conditions.
• Dementia and cognitive disorders.
• Osteoarthritis and chronic pain conditions.
• Urinary incontinence.
• Manage multiple chronic conditions.
• Order and interpret diagnostic tests and labs.
• Develop and manage individualized treatment plans.
• Collaborate with physicians, specialists, caregivers, and inter-disciplinary team members.
• Support patient goals while promoting independence, safety, and quality of life.
