CHRISTUS St. Vincent in Santa Fe, New Mexico, seeks a compassionate Nurse Practitioner for the Center for Healthy Aging. This full-time outpatient role focuses on geriatric care, promoting function and quality of life for older adults within a team-based setting.

The NP will perform wellness visits, manage chronic conditions, and collaborate with a multidisciplinary team to provide person-centered care guided by the 4Ms framework: Mentation, Medications, Mobility, and What Matters.

Formación

• Two years NP experience minimum.
• Active New Mexico NP license or ability to obtain one.
• Passion for caring for older adults and delivering patient-centered care.

Responsabilidades

• Provide comprehensive outpatient care for older adults, including annual wellness visits and preventive health screenings.
• Evaluate, diagnose, and treat acute and chronic conditions.
• Manage geriatric concerns such as dementia, osteoarthritis, urinary incontinence, and multiple chronic conditions.
• Order and interpret diagnostic tests and laboratory studies.
• Develop and manage individualized treatment plans.
• Collaborate closely with physicians, specialists, caregivers, and interdisciplinary team members.
• Support patient goals while promoting independence, safety, and quality of life.
