Description:Function independently to provide high quality patient care in a way that is collaborative and patient-centric.Care may occur in a variety of settings and by different modalities.This includes direct interaction with patients in the hospital or clinic, but also via telephone, video, or secure email when appropriate.Care will include gathering of history from the patient and others, performing physical exams, reviewing prior records, and assessing ancillary data.Utilize sound judgment, critical problem solving skills, and up-to-date medical information.Develop and monitor appropriate treatment plans.Communicate effectively with the patient and family when appropriate, as well as the care team.Provide efficient and compassionate care for both the patient and their family using a multi-disciplinary approach.Provide relevant education and counseling to patients and families to support care plans, treatment, and discharge.Collaborate with other healthcare team members to provide safe patient care.Communicate pertinent findings and information to the physician and care team.Assist collaborating physicians with management of patients.Ensure compliance with regulatory standards and practice metrics.Document in accordance with regulatory and care site guidelines.Promote the mission, vision, and values of the organization.Perform other duties as assigned.May be required to float to other departments based on business need.May be required to be placed on-call during a regularly scheduled shift.
