We are seeking compassionate and driven Nurse Practitioners to join our dynamic Clover Home Care teams in Atlanta, Georgia. This is a unique opportunity to provide comprehensive primary and supportive care directly in the homes of some of our most vulnerable members, helping to improve health outcomes and quality of life.
As a Clover Home Care Nurse Practitioner, you’ll join our Clover Care Visit (CCV) Team. The CCV team focuses on early intervention and transitional care, helping to set the foundation for long‑term health and reduce avoidable complications. You’ll work closely with our clinical insight platform, Clover Assistant, to support decision‑making and improve outcomes.

Final pay is based on several factors including but not limited to internal equity, market data, and the applicant’s education, work experience, certifications, etc.
Base salary range for this role is: $140,000 — $165,000 USD
