Job summary

Gilder Search Group is seeking Nurse Practitioners to perform in-home Annual Wellness Visits for Medicare members. Visits are conversational, consultative, and education-focused with no prescribing or treatment plans.

You can set your own schedule across weekdays and weekends, with a guaranteed daily rate and autonomous field-based work. Role includes conducting assessments, educating on preventive care, and ensuring documentation in the EMR.

Qualifications

• Active State NP license and board certification.
• Valid NPI number.
• Minimum 3 months wellness exam experience preferred.
• Comfort with EMR/documentation systems.

Responsibilities

• Conduct comprehensive in-home wellness assessments.
• Educate members on preventive care benefits.
• Identify gaps in care to reduce avoidable ER visits and hospitalizations.
• Complete documentation using provided EMR tools.
• Visits are face-to-face at members' homes.

What You’ll Do

• Visits are face-to-face and take place where members live-allowing for more personalized, impactful care.

Requirements

• Active State NP license and Board Certification
• Valid NPI
• Minimum 3 months of wellness exam experience preferred
• Comfort with EMR/documentation systems
• Ability to travel within a 30-50-mile radius
• Bilingual/Spanish Speaking a plus
• Give on average 2 days per week
