Family Resource Center in Findlay, OH seeks a Nurse Practitioner or Physician Assistant to join our team-based care model. The role focuses on comprehensive primary care, mental health integration, and coordinated referrals to community resources.

Applicants should have an accredited NP/PA program, OH licensure, and relevant board certification. A passion for diverse populations and cultural competence is essential, with clinic collaborations across disciplines and emphasis on patient education

Qualifications

• Accredited NP/PA program with clinical experiences across age groups.

Responsibilities

• Provide direct care services including health assessments, exams, diagnoses and treatments.
• Develop and implement individualized treatment plans based on patient needs and evidence‑based practice.
• Order, perform, and interpret diagnostic tests and labs.
• Prescribe medications and therapies within scope of practice.
• Provide preventive care, screenings, immunizations, and health risk assessments.
• Educate patients and families on disease prevention, medications, and healthy living.
• Coordinate referrals to specialists and community resources.
