Job summary

Chestnut Health in Illinois seeks a compassionate Nurse Practitioner to join our Physiatry team, delivering patient-centered rehabilitation care and improving functional independence. The NP will collaborate with therapists, nurses, and physicians to develop treatment plans, perform assessments, monitor progress, educate patients and families, and document care in electronic medical records.

Qualifications

• Eligible to work in the United States.
• Active nursing license in state of employment preferred or required.
• Current DEA license and CDS license preferred.

Responsibilities

• Collaborate with therapists, nurses, and other professionals to plan comprehensive rehab treatments.
• Conduct patient assessments, histories, exams, and evaluations for rehabilitation goals.
• Monitor progress and adjust treatment plans to improve function and independence.
• Participate in multidisciplinary team meetings and continuing education.
• Document evaluations, plans, progress notes, and discharge summaries in EMR.
• Educate patients and families about rehabilitation processes and post-discharge care.

Essential Duties & Responsibilities:

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs.
• Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions.
• Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence.
• Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills.
• Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems.
• Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care.

Qualifications:

• Eligible to work in the United States
• Actively licensed to practice in the state of employment preferred
• Current Controlled Substance License by start date preferred
• Current DEA License by start date preferred
• ANCC or AANP certification preferred
• Interest in working in Physiatry / Physical Medicine and Rehabilitation is a plus
• Experience in orthopedics is a plus
• Experience in physiatry is a plus
• Experience in skilled nursing is a plus

License/Certification:

• Certified Nurse Practitioner certification preferred
