Chestnut Health, a leading rehabilitation-focused healthcare organization, seeks a compassionate Nurse Practitioner to join our physiatry team in Rocky Mount, North Carolina. This patient-centered role collaborates with therapists and nurses to create and execute comprehensive rehab plans, aiming to restore function and independence.

Competenze

Licensed to practice as a Nurse Practitioner in NC.
Current Controlled Substance License.
Current DEA License.
ANCC or AANP certification.
Interest in Physiatry/PM&R.
Orthopedic experience is a plus.
Skilled Nursing experience is a plus.

Mansioni

Collaborate with therapists, nurses, and healthcare professionals to develop rehab plans.
Conduct patient assessments including histories, exams, and evaluations to set rehab goals.
Monitor progress and adjust treatment plans to promote independence.
Participate in multidisciplinary team meetings and continuing education to enhance skills.
Maintain documentation in EMR for evaluations, plans, progress, and discharge summaries.
Provide education to patients and families about rehab process and post-discharge care.

Essential Duties & Responsibilities:

Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs.
Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions.
Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence.
Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills.
Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems.
Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care.
