Job summary

Chestnut Health is seeking a Nurse Practitioner to join our Physiatry/PM&R team in Illinois. New graduates are encouraged to apply for a role that emphasizes collaborative care, patient-centered outcomes, and comprehensive rehabilitation.

You will work with therapists, nurses, and physicians to develop and implement treatment plans, conduct assessments, monitor progress, and educate patients and families about recovery and post-discharge care.

Qualifications

  • Eligible to work in the United States.
  • Active state license preferred.
  • Current DEA license preferred.
  • ANCC or AANP certification preferred.
  • Interest in Physiatry / PM&R a plus.

Responsibilities

  • Collaborate with therapists, nurses, and physicians to design rehab treatment plans.
  • Conduct patient assessments and evaluations to set rehabilitation goals.
  • Monitor progress and adjust plans to improve function and independence.
  • Participate in multidisciplinary meetings and ongoing education.
  • Document evaluations, plans, progress notes, and discharge summaries in EMR.
  • Educate patients and families about the rehab process and post-discharge care.

Essential Duties & Responsibilities:

  • Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs.
  • Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions.
  • Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence.
  • Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills.
  • Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems.
  • Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care.

Qualifications:

  • Eligible to work in the United States
  • Actively licensed to practice in the state of employment preferred
  • Current Controlled Substance License by start date preferred
  • Current DEA License by start date preferred
  • ANCC or AANP certification preferred
  • Interest in working in Physiatry / Physical Medicine and Rehabilitation is a plus
  • Experience in orthopedics is a plus
  • Experience in physiatry is a plus
  • Experience in skilled nursing is a plus

License/Certification:

  • Certified Nurse Practitioner certification preferred
