Active license to practice in Arizona.
Current Controlled Substances license.
Current DEA license.
ANCC or AANP Nurse Practitioner certification.
Desire to work in Physiatry/PM&R.
Orthopedic/PM&R experience is beneficial.

Responsibilities

Collaborate with therapists, nurses, and other professionals to develop rehabilitation plans and goals.
Assess patients, obtain histories, perform exams, and determine rehabilitation goals.
Monitor progress and adjust treatment plans to optimize function and independence.
Participate in multidisciplinary meetings to share knowledge and advance care.
Document evaluations, plans, progress notes, and discharge summaries in EMR.
Educate patients and families about rehabilitation processes and post-discharge care.

Essential Duties & Responsibilities

Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs
Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions
Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence
Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills
Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems
Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care
