Chestnut Health in Arkansas is seeking a Nurse Practitioner to join our physiatry team. You will collaborate with therapists, nurses, and physicians to develop and implement comprehensive rehabilitation plans for patients with diverse needs.

Qualifications

• Must be eligible to work in the United States.
• Active licensure in the state of employment preferred.
• Current Controlled Substance License by start date preferred.
• Current DEA License by start date preferred.
• ANCC or AANP certification preferred.
• Interest in working in Physiatry / Physical Medicine and Rehabilitation is a plus.
• Experience in orthopedics is a plus.
• Experience in physiatry is a plus.
• Experience in skilled nursing is a plus.

Responsibilities

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs.
• Conduct thorough patient assessments, including histories, examinations, and diagnostic evaluations, to determine rehabilitation goals and interventions.
• Monitor patient progress closely and adjust treatment plans to optimize functional outcomes and independence.
• Participate in multidisciplinary team meetings, case conferences, and educational activities.
• Maintain accurate documentation of evaluations, treatment plans, progress notes, and discharge summaries in EMR systems.
• Provide education and counseling to patients and families regarding rehabilitation and post-discharge care.

• Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions.
• Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence.
• Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills.
• Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems.
• Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care.
