Chestnut Health Systems in Rockwood, TN is seeking a Nurse Practitioner to join our physiatry team and deliver rehab-focused care. You will collaborate with therapists, nurses, and physicians to create and implement comprehensive treatment plans aimed at restoring function and independence.

Qualifications

Licensed to practice in the state employed.
Current Controlled Substance License required at start date.
Current DEA License required at start date.
ANCC or AANP certification.
Interest in Physiatry/Rehabilitation.
Experience in Orthopedics and/or Skilled Nursing is a plus.

Responsibilities

Collaborate with therapists, nurses, and other professionals to develop rehab plans.
Perform patient assessments including histories and physical exams.
Monitor progress and adjust plans to optimize outcomes.
Join multidisciplinary team meetings and trainings.
Document evaluations, plans, progress, and discharges in EMR.
Educate patients and families about rehab and post-discharge care.

Essential Duties & Responsibilities:

Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs.
Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions.
Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence.
Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills.
Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems.
Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care.
