Job summary

Chestnut Health in Georgia is seeking a Nurse Practitioner to join our physiatry team, delivering comprehensive rehabilitation care in a patient-centered setting. You will collaborate with therapists, nurses, and other professionals to assess patients, develop and adjust treatment plans, monitor progress, and educate patients and families about the rehab process and expectations.

Qualifications

  • Actively licensed to practice as a Nurse Practitioner in the state of employment.
  • Current Controlled Substance License (nice to have).
  • Current DEA License (nice to have).
  • Eligible to work in the United States.
  • Orthopedic experience (nice to have).
  • Physiatry experience (nice to have).
  • Skilled Nursing experience (nice to have).

Responsibilities

  • Collaborate with therapists, nurses, and other professionals to develop comprehensive treatment plans.
  • Conduct patient assessments, including medical histories and examinations, for rehabilitation goals.
  • Monitor progress and adjust treatment plans to optimize functional outcomes.
  • Participate in multidisciplinary meetings to enhance clinical knowledge.
  • Maintain accurate documentation of evaluations, plans, and progress in EMR.
  • Provide education and counseling to patients and families regarding rehab and discharge care.

Essential Duties & Responsibilities:
  • Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs.
  • Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions.
  • Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence.
  • Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills.
  • Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems.
  • Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care.
Qualifications:
