Job summary

Bristol Hospice in Georgia is seeking a compassionate Nurse Practitioner to provide expert end-of-life care and palliation to adults and children. You will work under a collaborating physician, within an interdisciplinary team, to deliver patient-centered services that honor comfort, dignity and family involvement.

Requirements include an accredited NP program and state licensure, with at least one year in hospice or palliative care.

Qualifications

• Graduate of an accredited nursing school and an accredited NP program.
• Current state RN and NP licensure.
• Minimum 1 year NP experience and 1 year hospice/palliative care experience.

Responsibilities

• Provide medical care and palliate terminal illness under physician supervision.
• Develop and manage individualized hospice care plans.
• Conduct history and physical exams and document findings.
• Complete hospice face-to-face visits per guidelines.
• Collaborate with interdisciplinary team.
