Visiting Nurse Association Health Group in New Jersey seeks a Nurse Practitioner to deliver in‑home primary care through the VPS program within the Parker Advanced Care Institute. You will work with an interdisciplinary team to assess, diagnose, and plan care for homebound, medically complex patients in the home setting.
The role requires a Master's in Nursing, NJ APN licensure with prescriptive authority, and national adult/geriatric NP certification.

As part of the Parker Advanced Care Institute at VNA Health Group, our interdisciplinary team of physicians, nurse practitioners, and physician assistants delivers comprehensive in‑home medical care, including routine visits, chronic disease management, telehealth, diagnostics, and wound care. Responsible for ensuring delivery of high quality care practices in the treatment of symptoms and promoting the highest quality of life for homebound patients and their families
Working indoors/outdoors with Client/Customer contact and clinical equipment and use of phone and computer; valid driver’s license and automobile insurance
In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $120,000 to USD $146,000. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add-on incentives, special clinical compensation, retirement plans, and all other benefits.
