VNACJ Community Health Center in New Jersey seeks a Family Nurse Practitioner to join our Pediatric and Primary Care teams in Freehold and Keyport. This full-time role provides comprehensive primary care, from acute to preventive services, focused on pediatric patients.

The ideal candidate has outpatient pediatric experience, strong communication skills, and a passion for supporting children and families. Bilingual English/Spanish is strongly preferred to meet our diverse patient needs.

Qualifications

• APRN licensure in NJ required.
• Master’s degree in Nursing from an NLN-accredited program.
• Certified Family Nurse Practitioner with prescriptive authority; CDS/DEA license.
• 2+ years of primary or ambulatory care nursing experience.
• Leadership experience or business/management degree preferred.
• Valid car and driver's license.

Responsibilities

• Evaluates, diagnoses, and treats acute and chronic conditions.
• Delivers preventive care and promotes patient wellness across the lifespan.
• Provide care within Pediatric and Primary Care departments in Freehold and Keyport.
• Assist with quality improvement and patient education.
• Collaborate with interprofessional team to support children and families.
• Maintain infection control and safety standards.
• Carry out supervisory roles in conjunction with leadership practices.

Position Summary

The Family Nurse Practitioner provides comprehensive primary care services to patients within our Pediatric and Primary Care departments in Freehold and Keyport, NJ, with a significant focus on pediatric care. This full‑time role is responsible for evaluating, diagnosing, and treating acute and chronic conditions, delivering preventive care, and promoting patient wellness across the lifespan. The ideal candidate will have outpatient pediatric experience, strong communication skills, and a passion for supporting children and families. Bilingual English/Spanish skills are strongly preferred to help meet the needs of our diverse patient population.

Principal Responsibilities

• Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co‑workers
• Provides palliative care or hospice face‑to‑face evaluation services to individual and family clients within established productivity guidelines
• Provides client teaching to individuals and families when appropriate
• Monitors quality and appropriateness of services provided
• Identifies new services or programs relevant to agency’s mission and philosophy
• Adheres to agency infection control and safety policies and guidelines of primary care regulatory agencies
• Supervises staff in conjunction with the organization’s managerial and leadership practices

Job Competencies & Minimum Qualifications

• Licensure as an Advanced Practice Nurse in NJ
• Master’s degree in Nursing from an NLN accredited program
• Certified as a Family Nurse Practitioner with prescriptive authority; maintain a CDS and DEA
• 2+ years of primary or ambulatory care nursing experience required
• Business or management degree or experience preferred with demonstrated leadership skills
• Car and valid driver’s license

Working Conditions/Physical Demand: Working indoors. Exposure to blood, body fluids and communicable diseases. Sitting, fine motor hand movement, standing and walking. Client/customer contact.
