Job summary

Qualifications

  • Graduate of an accredited School of Nursing and NP program.
  • Current nursing licensure in the state.
  • Master's degree with at least 1 year NP experience and 1 year hospice/palliative care experience.
  • Accreditation from an approved certifying body for advanced practice nursing.
  • Certification in a specialist area preferred (e.g., CHPN).
  • Licensed driver with insured vehicle as required.

For more information about Bristol Hospice, visit bristolhospice.com or follow us on LinkedIn.Our CultureOur culture is cultivated using the following values:Integrity: We are honest and professional.Trust: We count on each other.Excellence: We strive to always do our best and look for ways to improve and excel.Accountability: We accept responsibility for our actions, attitudes, and mistakes.Mutual Respect: We treat others the way we want to be treated.On an Average Day You Will:(includes, but not limited to)Provide medical care, palliate and manage terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician to adults and childrenComplete history and physical examinations by evaluating the patient's medical condition and health history, ordering and interpreting results from laboratory and diagnostic tests or procedures, diagnosing health conditions and documents the findings in the patient's medical recordManage health conditions and terminal illness by developing an individualized plan of care, prescribing medications and/or treatments, obtaining consultations or making referrals and coordinating hospice care servicesComplete the hospice face to face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility based on the hospice conditions of participation and the local coverage determinationsProvide clinical information to the hospice medical director for interpretation of eligibility for hospice recertificationMake recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice careReview clinical information, coordinate and oversee patient care in consultation with the hospice interdisciplinary team and other healthcare practitionersAssure that patient receives appropriate measures to control symptoms, through collaboration with interdisciplinary team membersReview and develop protocols for treatment and propose options for interventions based on the literature in collaboration with the collaborating physicianConsult the collaborating physician when the patient's plan of care is outside standardized practice and protocolsCollaborate with the interdisciplinary group to create, review and revise the patient's plan of careFulfill the obligation of requested and/or accepted assignmentsDemonstrate knowledge in communication and counseling patient/family in dealing with end-of-life issuesComplete all other duties as assignedRequirements:Must be a graduate of an accredited School of Nursing and accredited Nurse Practitioner ProgramMust have current nursing licensure in StateMust have a master's degree with a minimum of one (1) year Nurse Practitioner experience and a minimum of one (1) year hospice or palliative care experienceMust have accreditation from an approved certifying body for advanced practice nursing as required by the StateCertification in a specialist area preferred; (e.g. Hospice and Palliative Nursing (CHPN), Pediatrics or Geriatrics)Must be a licensed driver with licensed automobile that is insured in accordance withorganization/state requirements and is in good working orderJoin a Team that embraces the reverence of life!Bristol Hospice is an Equal Opportunity Employer and participates in E-verify.
