Job summary

Chestnut Health Company in Indiana is seeking a Nurse Practitioner to join our Physiatry team within PM&R, focusing on restoring function for patients with injuries, illnesses, or disabilities. New graduates are encouraged to apply, with mentorship and collaborative care from therapists and nurses to deliver patient-centered rehab.

You'll conduct assessments, help develop treatment plans, monitor progress, document in EMR, and educate patients and families about the rehab process.

Qualifications

  • Must hold a Certified Nurse Practitioner credential.
  • Desire to work in Physical Medicine and Rehabilitation (Physiatry).
  • Eligible to work in the United States.
  • Current DEA license and CS license are nice to have.

Responsibilities

  • Collaborate with therapists, nurses, and other healthcare professionals to develop comprehensive rehab plans.
  • Conduct thorough patient assessments to determine rehab goals and interventions.
  • Monitor patient progress and adjust treatment plans to optimize function.
  • Participate in multidisciplinary team meetings and educational activities.
  • Maintain accurate documentation of evaluations, plans, progress, and discharges in EMR.
  • Provide education to patients and families about rehab processes and post-discharge care.

Essential Duties & Responsibilities:
  • Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs
  • Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions
  • Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence
  • Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills
  • Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems
  • Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care
Requirements
Qualifications:
  • ANCC or AANP certification
  • Desire to work in Physical Medicine and Rehabilitation (Physiatry)
  • Eligible to work in the United States
  • Current Controlled Substance License (nice to have)
  • Current DEA License (nice to have)
  • Orthopedic experience (nice to have)
  • Physiatry experience (nice to have)
  • Skilled nursing experience (nice to have)
License/Certification:
  • Certified Nurse Practitioner (Required)
