Chestnut Health in Indiana seeks a Nurse Practitioner to join our Physiatry team, delivering comprehensive PM&R care and guiding rehabilitation goals for patients with mobility impairment. Qualifications

  • ANCC or AANP certification required.
  • Interest in Physical Medicine and Rehabilitation.
  • Authorized to work in the United States.
  • CS license and DEA license are preferred.
  • Experience in orthopedics or physiatry is a plus.
  • Nursing experience valued for rehab care.

Responsibilities

  • Collaborate with therapists and clinicians to develop rehab plans.
  • Conduct patient assessments and determine rehab goals.
  • Monitor progress and adjust treatment plans to improve independence.
  • Participate in multidisciplinary meetings and education sessions.
  • Document evaluations, plans, progress, and discharge summaries.
  • Educate patients and families about rehab process and outcomes.

Essential Duties & Responsibilities:
  • Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs.
  • Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions.
  • Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence.
  • Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills.
  • Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems.
  • Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care.
Qualifications:
  • ANCC or AANP certification
  • Desire to work in Physical Medicine and Rehabilitation (Physiatry)
  • Eligible to work in the United States
  • Current Controlled Substance License (nice to have)
  • Current DEA License (nice to have)
  • Orthopedic experience (nice to have)
  • Physiatry experience (nice to have)
  • Skilled nursing experience (nice to have)
License/Certification:
  • Certified Nurse Practitioner (Required)
