Job summary

Chestnut Health in New Jersey is seeking a Nurse Practitioner to join our physiatry team. This role emphasizes collaboration with therapists and nurses to design and execute comprehensive rehabilitation plans, with a focus on restoring function and independence.

You will conduct patient assessments, monitor progress, and document care in electronic records. We encourage new graduates and provide a patient-centered environment that supports work-life balance and ongoing professional growth.

Qualifications

Active state licensure to practice as a Nurse Practitioner.
Current DEA license required on start date.
Current Controlled Substance license required on start date.
ANCC or AANP certification.
Interested in Physiatry/PM&R specialty.
Orthopedic, Physiatry, or skilled nursing experience is a plus.

Responsibilities

Collaborate with therapists, nurses, and other healthcare professionals to develop and implement rehabilitation plans.
Conduct thorough patient assessments including history and exams to set rehab goals.
Monitor progress and adjust treatment plans to improve function and independence.
Participate in multidisciplinary meetings and educational activities.
Maintain accurate documentation in electronic medical records.
Educate patients and families on rehab process and post-discharge care.

Essential Duties & Responsibilities:

Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs.
Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions.
Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence.
Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills.
Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems.
Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care.
