Job summary

Premise Health Inc. is seeking a part-time Nurse Practitioner to deliver primary care to employees and their dependents at our Health Center in South Bend, IN. The role is scheduled to start in early January 2027.

You will conduct physical exams, manage preventive care, order tests, and collaborate with physicians to develop care plans. A current state NP license, national certification, and BLS are required.

Qualifications

• Current state RN license to practice.
• Nurse Practitioner license/certificates to practice.
• Federal DEA license for practice where applicable.
• ANCC or AANP national certification.
• Master’s Degree in Science of Nursing (MSN).
• Current BLS/CPR certification; ACLS may be required by contract.

Responsibilities

• Conducts physical exams and preventive health measures per guidelines.
• Performs procedures such as injections, suturing, wound care, and infection management.
• Develops patient care plans, records progress, and ensures continuity of care.
• Educates patients on treatment plans and health maintenance.
• Orders and interprets diagnostic tests to assess clinical conditions.
• Collaborates with physicians and healthcare teams for comprehensive care.
• Prescribes or recommends treatments, including medications and therapies.
• Refer patients to specialists or physicians as needed.
• Maintains patient health records with accurate documentation.
• Educates and coaches nursing staff on best practices.
• Performs other duties as assigned.

As a Part -Time (28 hours per week) Nurse Practitioner (NP), you'll provide care to client employees and their dependents in our Health Center located in South Bend, IN.

What You’ll Do

• Conducts physical exams and preventive health measures per guidelines
• Performs procedures such as injections, suturing, wound care, and infection management

• Orders and interprets diagnostic tests to assess clinical conditions
• Collaborates with physicians and healthcare teams for comprehensive care
• Prescribes or recommends treatments, including medications and therapies

• Maintains patient health records ensure accurate and up-to-date documentation and records
• Educates and coaches nursing staff on best practices
• Performs other duties as assigned

What You’ll Bring

• Maintains state RN licenses/certificates to practice
• Maintains Nurse Practitioner licenses/certificates to practice
• Maintains State Prescriptive Authority (if applicable)
• Federal DEA license (matching each state of practice as applicable)
• State Narcotic/Controlled Substance license if required in your state (Alabama, Connecticut, DC, Delaware, Hawaii, Idaho, Illinois, Indiana, Iowa, Louisiana, Maryland, Massachusetts, Michigan, Nevada, New Jersey, New Mexico, Oklahoma, Rhode Island, South Carolina, South Dakota, Utah, Wyoming, Puerto Rico) (New York ROPES registration)
• Current national certification (ANCC or AANP required)
• Master’s Degree in Science of Nursing (if Master Degree in Science certificate does not state Nursing on it then a copy of transcripts is required)
• Current certification with hands-on training in AHA, ARC or equivalent Basic Life Support for health care providers is required; Advanced Cardiac Life Support may also be required based on contract scope of services
• At least three years of clinical experience as a Nurse Practitioner in an ambulatory care, occupational health, family practice or emergency department setting
