Chestnut Health is seeking a compassionate Nurse Practitioner to join our Physiatry team. You will collaborate with therapists and nurses to develop rehab plans, conduct thorough patient assessments, and monitor progress to restore function and independence.

Qualifications

Active nursing license in the state of employment.
DEA license required by start date.
ANCC or AANP certification for PM&R nursing practice.
Certified Nurse Practitioner credential required to apply.
Interest in Physiatry/PM&R and rehab-focused practice.
Orthopedic experience is a plus; Physiatry experience preferred.
Skilled nursing experience is a plus.

Responsibilities

Collaborate with therapists, nurses, and other healthcare professionals to develop treatment plans for rehabilitation patients.
Conduct comprehensive patient assessments to determine rehabilitation goals.
Monitor progress and adjust treatment plans to improve function and independence.
Attend multidisciplinary meetings to enhance clinical knowledge.
Document evaluations, plans, progress notes, and discharge summaries in EMR.
Educate patients and families about rehabilitation process, outcomes, and post-discharge care.

Essential Duties & Responsibilities:

Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs.
Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions.
Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence.
Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills.
Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems.
Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care.
