Qualifications

Active licensed Nurse Practitioner in the state of employment.
Current Controlled Substance and DEA licenses required on start date.
ANCC or AANP certification is required.
Interest in Physiatry/PM&R and rehabilitation.
Orthopedic or skilled nursing experience is a plus.

Responsibilities

Collaborate with therapists, nurses and clinicians to develop rehabilitation plans.
Conduct patient assessments, histories, exams and diagnostic evaluations.
Monitor progress and adjust treatment plans to improve function and independence.
Participate in multidisciplinary meetings and continue medical education.
Document evaluations, plans, progress notes and discharge summaries in EMR.
Educate patients and families about rehabilitation expectations and post-discharge care.

Essential Duties & Responsibilities:

Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs
Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions
Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence
Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills
Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems
Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care
