CompassionCare Hospice in Reno, NV is seeking a compassionate Nurse Practitioner to join our team. You will perform patient assessments, manage symptoms, and coordinate hospice services to align with a patient’s goals and values.

Qualifications

  • Master's degree in nursing and an active NP license.
  • 5+ years nursing experience.
  • Prescriptive authority and DEA registration if applicable.

Responsibilities

  • Provide compassionate medical care and symptom management.
  • Perform assessments, order/interpret tests, and develop care plans.
  • Prescribe medications and coordinate hospice services.
  • Document visits for eligibility and recertification.
  • Collaborate with physicians and IDT to align care with patient goals.

As a Nurse Practitioner You will:
  • Provide compassionate medical care and symptom management
  • Perform assessments, order / interpret diagnostic tests, and develop care plans
  • Prescribe medications, manage treatments, and coordinate hospice services
  • Conduct and document face-to-face visits for eligibility and recertification
  • Collaborate with physicians and IDT team to align care with patient goals.
  • Participate in care planning and coordinate with team and external providers
  • Maintain accurate clinical documentation and ensure compliance regulation
  • Educate patients & families on medical, emotional, and spiritual needs
  • Contribute to quality improvement efforts and professional development
  • Serve as a clinical resource and liaison to community partners
Responsibilities
  • Facilitate new patient consultation and health history review
  • Perform physical examinations and screenings of patients
  • Order and administer tests including bloodwork
  • Review and interpret test results
  • Diagnose health problems
  • Detect changes in patient condition
  • Administer medications
  • Operate medical equipment
  • Recommend further treatment options or future appointments if needed
  • Provide referrals to other practitioners when necessary
  • Provide after-care/ongoing care information and resources to patients
  • Ensure patient records are kept up to date including test results, immunizations, condition changes, etc.
  • Deliver quality, physician-level care while complying with company policies, safety regulations, and productivity metrics
  • All other duties as assigned
Qualifications
Education
  • Master's degree in nursing
  • Current state nurse practitioner license required
  • 5+ years nursing experience required
Qualifications
  • Strong knowledge base of HIPAA and OSHA required
  • Strong interpersonal communication skills, both verbal and written
  • Patient, compassionate nature
  • Ability to operate as part of a team, alongside physicians and other medical staff
  • Proficiency in medical instruments and equipment
  • Proficiency in computer-based data management programs and information systems preferred
  • Prescriptive authorization required
Work Requirements
  • Active unrestricted license to practice in state
  • Certificate/license as a Nurse Practitioner to meet individual state practice act
  • Current DEA registration if applicable
