Job summary

VNAcare in Murrieta, CA is seeking a Nurse Practitioner for a Per Diem daytime shift to join our hospice team. The NP will provide compassionate, interdisciplinary care to patients in their homes and communities, ensuring adherence to hospice philosophy and regulations.

Requirements include active California RN and NP licenses and at least one year nursing experience; hospice/palliative care is preferred. Travel for home visits and flexible scheduling are offered to support work-life balance.

Qualifications

• Current California RN license is required.
• Current California Nurse Practitioner license is required.
• Minimum one year of professional nursing experience is required.
• Hospice or palliative care experience is preferred.

Responsibilities

• Provide comprehensive hospice nursing care and clinical assessment.
• Collaborate with interdisciplinary team to develop care plans.
• Document care activities accurately and timely.
• Educate patients and families on hospice principles and goals of care.
• Manage schedules and coordinate home visits for recertification.

The Nurse Practitioner position is open in our Murrieta location and surrounding areas. This is a Per Diem position during our day shift.

Requirements of the Nurse Practitioner

• Current Registered Nursing license in the State of California.
• Current Nurse Practitioner license in the State of California.
• Minimum of one year professional Nursing experience is required.
• Hospice/Palliative Care experience preferred.

Nurse Practitioner Duties Include

• Demonstrates an ability to work within an interdisciplinary setting.
• Demonstrates an understanding of Hospice philosophy, principles of death and dying.
• Compliance with accepted professional standards and practices.
• Demonstrate excellent observation, assessment, nursing judgment and communication skills.
• Ability to provide accurate written documentation in a timely manner.
• Self-directed with the ability to work with little supervision.
• Demonstrates ability to efficiently schedule home visits of patients needing a face to face visit for recertification.
